A surgical method and tool for expanding an initial osteotomy () to receive a bone implant (). An osteotome () having a tapered working end () is inserted into the initial osteotomy (). The initial osteotomy () is enlarged by simultaneously rotating and pushing the working end () of the tapered osteotome () into the osteotomy (). When rotated in one direction the burnishing edges () concentrate the pushing and rotational force in outward normal and tangential component forces against the interior surface of the osteotomy () to incrementally expand the osteotomy () with little to no removal of bone material (). When rotated in the opposite direction the burnishing edges cut the interior surface of the osteotomy. Progressively larger tapered osteotomes () are used until an osteotomy () of predetermined size is achieved.

Abstract:
Tools and methods for expanding a precursor hole in a host material to receive a fixture. The precursor hole is enlarged by a rotary tool having helical flutes and interposed lands. The flutes have a negative rake angle. The lands each have a working edge that cuts the host material when the tool is rotated in a cutting direction, and that condenses the host material when the tool is rotated in a densifying direction. The body of the rotary tool has a stopper section that plugs the hole when a certain depth is reached. When the tool is used with a copious wash of irrigating fluid at or below the necessary depth, hydraulic pressure builds inside the precursor hole. The hydraulic pressure can be advantageously exploited in cutting mode to autograft a slurry of host material particles into the sidewalls of the hole and create an incipient densifying crust.

A rotary osteotome for deep reach applications. The body of the osteotome has a tapered end that supports helically spiraling flutes. Substantially margin-less (without margin) working edges are interleaved between the flutes to provide compaction action when rotated in a non cutting direction. The body also has an elongated cylindrical stopper section. An irrigation conduit passes through the center of the stopper section and emerges at a plurality of outlet orifices that function as independent nozzles for irrigating fluid. The outlet orifices are generally elliptical in shape and spaced around the body to maintain balance. The irrigation conduit has a main trunk that opens to a flow splitter, which in turn divides the flow of irrigating fluid into substantially equal branches. Each branch is angled at an acute trajectory relative to the longitudinal axis, in the direction of said apical end, between about 10 and 45.

Abstract:
An anchor to be screwed into a hole and self-lock therein with high initial stability. The anchor has an aggressively-threaded, self-tapping apical end. A central region of the anchor is formed with a plurality of helical flutes and intervening lands. Each land carries a condensing edge. The condensing edges are configured to apply a circumferentially sweeping compressive strain to the interior surface of the host material while the anchor is being screwed into position. A coronal end of the anchor includes a corking feature to mitigate mushrooming around the perimeter of the hole. A helical groove intersects the condensing edges and flutes. The helical groove has a variable pitch and a decreasing depth which functions to squeeze and displace trapped host material thereby enhancing stability and other benefits. In bone applications, the groove hosts material chips which promote and enhance healing.

A surgical tool assembly for forming holes in bone at precise locations and to controlled depths. The drilling tool has a leading apical tip and a shank. The shank includes an annular groove at a predetermined distance from the apical tip. An adjustable stop gauge is coupled to the shank. The stop gauge includes an indexing adapter which telescopically supports a tubular collar. The collar includes an integrated impeller that accentuates irrigation of the treatment site during surgical procedures. The adjustable stop gauge may be used in combination with a guided surgery jig. The jig includes a guide bushing having a generally semi-cylindrical alignment valley adapted to receive the spinning collar of the stop gauge. The alignment valley includes an elevated internal abutment step having a semi-annular surface adapted to engage the collar when the apical tip reaches a predetermined penetration depth in the bone.
